HCM has been providing Disability Support programs since 1991.
We are an accredited provider for the National Disability Insurance Scheme (NDIS) and offer a range of services to individuals with a disability, including, case management, supported accommodation, life skills, in home support, daily living, social skills and community participation.
What is NDIS?
The National Disability Insurance Agency (NDIA) is an independent statutory agency, whose role is to implement National Disability Insurance Scheme (NDIS) which will support a better life for those in our community with a disability. The NDIS will provide peace of mind across Australia through supporting individuals to receive appropriate care and supports, through providing information to individuals and carers about available services.
The NDIS will ensure individuals can get the supports they need and achieve their goals, whilst also having choice, flexibility and control over their supports.
Hobart City Mission is an accredited provider of NDIS. We understand that every person is different, and therefore their needs and goals are all unique. We will work with you to ensure our services are flexible and suitable, to meet your needs.
Our staff will work with you and your family and/or carers to grow a strong, trust-based relationship.
Our NDIS Services
Social and Community Participation Program
We can provide individual one-on-one home support programs to enable you to achieve greater independence and to improve your capacity to participate fully in community life. This support is carried out by our professionally trained and family friendly disability support staff.
This support can include:
- Personal support
- Support with transitions
- Assistance with personal activities
- Assistance with daily tasks/shared living
- Supported development of life skills
- Support with household tasks
- Aid with social and community participation
- Assistance with Specialised Disability Accommodation
When you receive your NDIS plan, it can be very overwhelming – you have many choices to make, about things you may have never heard of.
Our Support Coordination specialist will assist you to understand your plan, to access appropriate supports and services. This is all done with the aim to help you achieve the goals set out in your plan to participate fully in the community.
Our Support Coordination services will assist you with tasks such as:
- Linking you with the right provides to meet your needs
- Assistance in sourcing providers
- Coordination of a range of funded and mainstream supports, whilst building on informal supports
- Resolving points of crisis
- Developing your resilience in your own network and community
Supported Independent Living (Group Homes)
Hobart City Mission has four group homes that provide 24 hour on-site, supported accommodation for residents with physical or intellectual disabilities. These homes are located around the greater Hobart area, and provide an opportunity for supported independent living.
An important principle of our supported accommodation program is to encourage family involvement in the planning of daily support for their loved one. HCM is committed to providing a home environment for Group Home participants who are very much part of the ‘HCM Family’
Non-NDIS Disability Services
Individual Support Packages
Individual Support Packages (ISP) provides assistance with, but not limited to, coordination and transition planning, personal care and support to complete everyday tasks, support to participate in a leisure activity or pre-vocational courses, equipment, home modifications, community access (day support) and/or, other services or goods required by a person to meet disability related need. This service is for individuals who are yet to transition to the NDIS.
